For applicants who were granted a Utah Associate Educator License or endorsement through License Equivalency Review based on:

An out-of-state license 

An international license 

A professional or occupational license (e.g., DOPL CSW or LCSW)

Or for those adding the CCR endorsement to their School Counselor License 

Use this application to submit documentation that you have completed the requirements to upgrade an Associate Educator License  or endorsement to professional. 

Do not use this application if you: 

Want to upgrade an associate endorsement that was not granted through License Equivalency Review. Use the Endorsement Only Application in SM Apply. 

Have an Associate Educator License from Utah and have never held a license in another state or country.  Contact your APPEL coordinator or your University program. 

Are upgrading an LEA-Specific license to an Associate or Professional License. Contact USBE Educator Licensing at (801) 538-7500.
